Central Peninsula Health Foundation has established a sustainable partnership with the Kenai Peninsula Food Bank to create a standardized process for providing eligible patients with a three-to-four-day supply of nutritious, shelf-stable food at discharge; the Food for Wellness program will provide short-term nutritional support to patients identified as food insecure through SDOH screening, facilitate a safer transition home, improve the patient experience, and support the hospital’s commitment to addressing health equity and Social Determinants of Health within the community.
